Before you use these: the Word and Excel templates contain bracketed fill-in fields such as practice name, dates and the compliance officer’s name, plus sample rows in the exclusion log. Replace them with your own details before putting a document into use.
Fraud & Abuse Prevention Guide

HIPAA Risk Assessment Template
